Community Assessment

Foundation Assessment

For families where a full parenting capacity assessment isn’t required, we can provide support with one of our Parent and Child foster carers, supported by specially trained Supervising Social Workers.

Includes:

Initial planning meeting with Local Authority Social Worker, Supervising Social Worker, Parent, and Foster Carer – Agreement forms, and Support and Development Plan completed

Weekly recording completed by Foster Carer electronically on parent and child specific recording form

Foster carer will provide daily progress support to parent in accordance with the Support and Development Plan

Foster Carer access to monthly Parent and Child support groups

Foster Carer has completed Parent & Child training

Supervising Social Worker completes monthly supervision with Foster Carer

Comprehensive Assessment

For families where a full parenting capacity assessment is needed, we can provide support with one of our Parent and Child foster carers, supported by specially trained Supervising Social Workers and Therapeutic Practitioners.

All from Foundation plus:

Full ParentAssess assessment completed by a Social Work Qualified and ParentAssess trained Designated Independent Assessor
Designated Assessor completes Strengths and Concerns – this will feed into the ongoing progression of the Support and Development Plan
Weekly Support and Development meeting between Designated Assessor and Parent
Designated Assessor will complete a minimum of three observations of child and parent
Full court approved parenting capacity report completed by the Designated Assessor
Brief weekly progress report completed by Designated Assessor and sent to Local Authority Social Worker
Access to bespoke therapeutic support for Parent
Supervising Social Worker completes fortnightly supervision with Foster Carer
